End of Life Care for the LGBTQ+ Community

SAGECare is a division of SAGE the country’s oldest and largest non-profit organization dedicated to improving the lives of LGBTQ+ older adults. SAGECare provides LGBTQ+ competency training and consulting on LGBTQ+ aging issues to service providers.

Constellations of Care is a SageCare Platinum credentialed organization and is also LGBTQ+ owned/operated, dedicated to care that honors identity and personhood through the end of life and beyond.

I had become aware the terrific educational information and beautiful and helpful messages around life & death and dying that she was putting out into the world on Instagram and through her website and newsletter, Constellations of Care.
