Pre-Planning Sessions

Between two sessions we will complete your end of life plan, including medical directives, care directives, funeral wishes, and organizing materials for easy access of loves ones. A pre-planning packet and state-specific medical directives are included, and guidance is provided on how to complete both in person and between sessions.

Constellations of Care is committed to LGBTQ+ inclusion and advocacy at the end of life, and all materials and sessions will have a focus on advocating for the protection of identity throughout life and beyond it.

Legacy Building

Constellations of Care supports Legacy building through life review, story telling, legacy projects and portraiture. Legacy services are determined by the vision of the person requesting them. In what ways would you like for your memory to live on in the world?

Life review is a process in which we will sit together for an hour at a time, over six weeks (seven sessions in total) using prompted questions to elicit your life stories and record them. I can assist with transcribing, or simple audio/video recording. Ongoing sessions can be booked for execution of a creative legacy project.

Legacy Package (Weekly one hour sessions for 7 weeks) | $450

Workshops

The Spread Care workshops series is for death workers (doulas, midwifes, companions, etc.), caregivers, and anyone that supports people through serious illness and end of life.

These workshops are hosted by Kate Muehlemann Cataldo, a death doula and Certified Hospice and Palliative Care Nurse (CHPN) with 20+ years experience in nursing and health care. She has worked in hospice for the past 7 years as both a Home Hospice RN, Hospice Educator, and LGBTQ+ Liaison for a hospice agency.
